CANCELLATION/ NO SHOW POLICY
DOCTOR APPOINTMENTS, EVALUATIONS AND THERAPY
We understand that there are times when you must miss an appointment due to emergencies, or workor family obligations. However, when we are not notified of your need to change an appointment time, we still reserve your appointment time slot. This may prevent another patient from receiving much needed care. To best serve all our patients we have the following policy:
*CANCELLATION/ NO SHOW POLICY FOR DOCTOR APPOINTMENTS AND OTHER EVALUATIONS.
If an appointment is not cancelled or rescheduled at least 24 hours in advance, you will be charged a fee of $35.00. This fee will not be covered by your insurance company.
*LATE ARRIVAL DOCTOR APPOINTMENT OR EVALUATIONS.
We understand that delays can happen, however we must maintain an on-time schedule for the doctors as well as other patients. If a patient arrives 15 minutes past their scheduled time for an examination, we may need to reschedule the appointment. Please contact us to let us know you are running late, and we will let you know if accommodation can be made.
*LATE ARRIVAL FOR THERAPY SESSIONS
We understand that delays can happen, however it is best to call our office to let us know that you are running late. It is better to come for some of the appointment than not at all. If you cancel your appointment with less than a 24-hour notice, you will be charged the cancellation fee.
* MISSED APPOINTMENTS WITHOUT NOTIFICATION
In the event of missing an appointment without providing further explanation for the absence, entitlement to a makeup session is forfeited. However, in cases of emergency where immediate communication is not feasible, doctors will be duly notified for review.
*VIRTUAL SESSIONS
We provide virtual appointments for vision therapy to ensure uninterrupted access to our services for those who cannot attend in-person sessions and to accommodate scheduling needs.
*MAKE UP SESSIONS
Sessions must be made up within 2 weeks of cancelled appointments. In the event that a makeup session is scheduled and subsequently canceled or missed by the patient, it is hereby established that no further makeup session will be arranged. The missed session will be deemed forfeited, and no exceptions to this policy will be entertained.
*EXTENDING SESSION WEEKS
If you need to cancel an appointment due to unforeseen circumstances such as illness, being out of town, or other scheduling conflicts with work, school, or other appointments, our admin staff will help you schedule a suitable makeup session. Should you wish to extend a week to your program, please note the following policy:
- 12-week vision therapy programs, one extra week can be added.
- 24-week vision therapy programs, two extra weeks can be added.
* ALL THERAPY IS TO BE COMPLETED BY THE END DATE OF YOUR AGREEMENT.
We understand that unexpected circumstances may arise, but we kindly request your compliance with the duration of your vision therapy program. Consistent attendance ensures optimal progress and successful outcomes. Your commitment to attending scheduled sessions
is crucial in achieving the best results for your visual health.
